From Justin

Sermorelin is a short-acting GHRH fragment that pushes your pituitary to release your own growth hormone. The theory is real; the bodybuilding benefit is probably less than you’ll want. For overall health as part of hormone optimization treatment, Sermorelin has a use case.

Overview

Sermorelin is a synthetic 29-amino-acid fragment of human growth hormone-releasing hormone used to stimulate endogenous growth hormone release via the pituitary GHRH receptor. It has been studied mainly for diagnostic use in childhood growth hormone deficiency and, more historically, in adult growth hormone insufficiency; it is not an FDA-approved bodybuilding drug. Human evidence for performance/body-composition use is thin compared with clinical growth hormone replacement, and the peptide’s short action makes timing and product quality important concerns.[^1]

Pharmacokinetics & Mechanism

  • Class: Synthetic growth hormone-releasing hormone (GHRH) analog / growth hormone secretagogue peptide
  • Mechanism of action: Sermorelin binds the growth hormone-releasing hormone receptor on pituitary somatotrophs, activating the adenylate cyclase/cAMP pathway and increasing pulsatile endogenous growth hormone secretion. It does not supply exogenous growth hormone; it works upstream by stimulating the patient’s own pituitary output. Published review literature describes it as a therapeutic/diagnostic GHRH analog, and PubMed indexing specifically labels it as pharmacologically related to growth hormone and GHRH activity (PubMed 18046908).
  • Target(s): Primary target: GHRH receptor (GHRHR) on anterior pituitary somatotrophs. Downstream: cAMP/PKA signaling and increased GH pulse secretion; secondary downstream effects occur through hepatic IGF-1 production. It is not a GH receptor agonist and does not directly activate IGF-1 receptor signaling.
  • Route(s) of administration: Subcutaneous injection is the standard route in the literature and in practice for the peptide form. Intravenous administration is described in older pharmacokinetic studies of related GHRH analogs, but sermorelin use itself is overwhelmingly subcutaneous in clinical/community settings. Oral, transdermal, and intranasal use are not established as reliable routes.
  • Onset: GH release occurs rapidly after administration, typically within minutes, but the exact onset for sermorelin in modern human studies is not well established in a single standardized way. For coaching purposes: expect an acute pituitary GH pulse rather than a slow depot effect.
  • Half-life: Short plasma half-life; exact human half-life for sermorelin is not consistently standardized across accessible sources and depends on assay and formulation. Related GHRH-analog literature shows that the native/short GHRH fragment class is rapidly cleared, while D-amino-acid substitution or PEGylation can extend exposure; these longer-acting variants are different compounds and should not be confused with sermorelin (PubMed search results on GHRH analog PK).
  • Metabolism / clearance: Cleared rapidly by enzymatic peptide degradation and renal handling typical of short peptide hormones; the literature on related GHRH analogs indicates dipeptidyl peptidase activity and other peptidases materially affect clearance, and modifications that resist degradation prolong half-life. Exact human metabolic clearance data for sermorelin itself are not well established in the accessible primary sources reviewed here.
